At Burwood Health, we are committed to protecting your privacy and handling your health information with care, in accordance with the Privacy Act 2020 and the Health Information Privacy Code 2020.

1. Purpose of Collection

We collect your personal and health information to:

  • Provide you with high-quality health care services

  • Maintain accurate medical records

  • Meet legal and funding obligations

  • Communicate with you about your care

  • Conduct quality improvement, teaching, and research (only with consent or in accordance with the law)

2. What We Collect

  • Medical history and treatment information

  • Test results, referrals, and other clinical data

  • Health insurance or funding eligibility information (e.g., Community Services Card)

We collect this information directly from you wherever possible, or from other health providers when required for your care.

3. Use and Disclosure

We will only use or disclose your information for purposes directly related to your care or where required by law. This may include sharing information with:

  • Other health providers involved in your care

  • Health agencies and funders such as Te Whatu Ora (Health New Zealand)

  • ACC, laboratories, or pharmacies

  • Our contracted service providers (e.g., for IT, billing, or secure storage)

We do not share your information for marketing or unrelated purposes without your consent.

4. Storage and Security

Your health information is stored securely, whether in physical or electronic form. We take reasonable steps to:

  • Protect your data from misuse, loss, or unauthorised access

  • Use secure systems and passwords

  • Limit access to only those staff who need it to do their jobs

You have the right to:

  • Request correction of any inaccurate or incomplete information

Burwood Health is contributing to, and accessing healthcare information from HealthOne –  

 What is HealthOne?

 HealthOne is a South Island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information. HealthOne adheres to the principles of the Privacy Act 2020 as well as the Rules set out in the Health Information Privacy Code 2020.  Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested.  Privacy auditing is used to check that only those directly involved in your care are accessing your information.  To find out more about HealthOne please visit https://healthone.org.nz/.  Please note that you are entitled to restrict the sharing of your healthcare records by contacting 0508 837 872 or emailing HealthOne.privacy@pegasus.health.nz”
