There are many benefits to enrolling with a General Practice.  Enrolling is free.

As an enrolled patient at Burwood Health you will be able to access a range of services, including:

  • GP and nurse consultations
  • Free consults for children under 14 years
  • Repeat prescription requests
  • Access to online health portal (Manage My Health)
  • Health Improvement Practitioner and Health Coach services
  • Vaccinations and health screening reminders
  • and so much more

CRITERIA FOR ENROLMENT:

To enroll as a new patient at Burwood Health you are required to meet the following criteria:

  • Are not currently enrolled with a medical centre in the local area
  • New Zealand Citizen
  • Permanent New Zealand Resident
  • Work Visa Holder

  • Dr Intan Md Tahir
  • Dr Gort Stephan
  • Sarah Clark (Nurse Practitioner)

ENROLLMENT PROCESS:

To begin the enrollment process, please contact our reception team on 03 383 0219.  You will be offered an appointment time to meet with our reception team to complete the Pegasus Enrollment forms.

WHAT TO BRING TO YOUR ENROLLMENT APPOINTMENT:

  • Proof identity (Drivers License or Passport)
  • Proof of Work Visa (if applicable)
  • Utility bill with your current home address

 

It is a requirement of the practice to view all identification to complete the enrollment process.

Once you have completed the enrollment process with our reception team, they will organise your first appointment at the practice.

** Please note that your enrollment appointment and first consultation with your GP cannot be on the same day **

 

YOUR FIRST APPOINTMENT AT BURWOOD HEALTH:

During your first appointment with Burwood Health, you will spend time with both a Practice Nurse and your enrolled Doctor.

We like to spend the time with you to ensure you receive the best care we can deliver.

WHAT TO EXPECT ON YOUR FIRST VISIT:
  • The practice’s Health Coach performs basic checks on your ‘vitals’ – blood pressure, pulse, height, blood sugar levels (if required)
  • Time for you to discuss your health history and your medical needs with our team

 

FEES FOR YOUR FIRST APPOINTMENT

The cost of your initial consultation is more than a standard consultation fee, however, in this appointment our clinical team spend time reviewing your medical history and ensuring we have all your information correct.

It is the policy of the practice that your initial consultation must be prepaid.

Community Service Card (CSC) Holders                                    $45

Non-Community Service Card Holders                                      $100

 

FURTHER APPOINTMENTS AT BURWOOD HEALTH:

Once you have completed your initial consultation with our clinical team, you will be able to book further appointments via your Manage My Health app (speak to our team when you enroll about joining Manage My Health), or by contacting our receptionists.
